Only antipsychotic proven effective for treatment-resistant schizophrenia
Unique receptor profile (D4, serotonin, muscarinic, histamine)
FDA Indications
Treatment-resistant schizophrenia (failed ≥2 antipsychotics)
Reduction of suicidal behavior in schizophrenia/schizoaffective disorder
Mechanism of Action
Weak dopamine D2 blockade → fewer EPS
Strong serotonin 5-HT2A antagonism
High affinity for D4, muscarinic, adrenergic, histamine receptors
Explains both efficacy and side effect burden
Black Box Warnings

Critical Safety Concerns
Agranulocytosis (severe neutropenia)
Increased mortality in elderly with dementia-related psychosis
Seizures (dose-related)
Myocarditis and cardiomyopathy
Severe orthostatic hypotension, bradycardia, syncope
Severe constipation / paralytic ileus

Baseline ANC ≥ 1500/µL (≥1000/µL for benign ethnic neutropenia)

Cardiac & GI Monitoring
Cardiac Monitoring
Baseline EKG, cardiac history, troponin/CRP if indicated

Watch for constipation, treat aggressively (laxatives, stool softeners)

Sedation

Sialorrhea (drooling)

Serious Adverse Effects & Clozapine Levels
Beyond Black Box Warnings
  • Myocarditis / cardiomyopathy
  • Pulmonary embolism, DVT
  • Rare: Neuroleptic malignant syndrome (lower risk than other antipsychotics)
Significance of Clozapine Levels
Therapeutic range:
  • Efficacy: ≥ 350 ng/mL typically needed for response
  • Optimal range: 350–600 ng/mL
Toxicity risk:
  • 600 ng/mL → higher risk of seizures and sedation
  • 1000 ng/mL → strongly associated with adverse effects
When to check levels:

Suspected non-response at moderate dose

Drug–drug interactions (e.g., fluvoxamine ↑ levels; smoking ↓ levels)

Changes in smoking status (CYP1A2 induction by smoking lowers clozapine levels)
Clinical pearl: Always interpret levels with patient's clinical response — not all patients respond the same way to identical concentrations.
Titration & Dosing
Start 12.5 mg once or twice daily
Increase slowly (25–50 mg/day) to reduce risk of hypotension & seizures
Target dose: 300–600 mg/day (sometimes up to 900 mg/day)
Clinical Pearls
Considered gold standard for suicidality in schizophrenia
Nonadherence common due to side effects & monitoring burden
Requires strong provider–patient alliance for compliance
Avoid abrupt discontinuation (risk of cholinergic rebound, psychosis relapse)
Case Example
Patient with schizophrenia, failed risperidone and olanzapine → started on clozapine.
